Annual Employee Benefit Enrollment Survey Available Until December 2nd

Each October, you have the opportunity to enroll in or change many of your employee benefits for the following year. This year's benefit enrollment period was from October 3 - 28th. UW System continually works to improve the resources available to employees during this period. Please respond to the online fall enrollment survey so we can better understand your experience during this past October's enrollment period.

Reminder: If you want to cancel your health insurance, EPIC Benefits+, VSP Vision Insurance, Anthem DentalBlue or Dental Wisconsin coverage for 2012, you must submit an application to your benefits office no later than December 1, 2011. All forms are available at: http://uwservice.wisc.edu/forms.php.
