* Demographic Information Request

Annually, UW System institutions report certain demographic information in order to comply with federal Affirmative Action regulations.

These regulations require us to collect demographic information regarding job applicants and employees and to regularly provide employees with the opportunity to self-identify as an individual with a disability and/or protected veteran.

Submission of this information is voluntary and is treated as confidential employee information.

Should you decide not to self-identify at this time, you may do so at any time in the future.

Please note: If you need further assistance with these items or need a reasonable accommodation to perform your job duties, please contact your Affirmative Action Officer.
